The speaker/moderator for this session will help provide relevant continuing education for attendees at the 2019 Annual Meeting. The session is scheduled for Weds 10/30 at 8:30 AM.
Session description:
The presenters will explore the roles of mental health and social determinants of health in the management of a population's total health. We will explore how emerging sources of data and information have led to a renewed focus on managing mental health conditions and social determinants of health. Speakers will address current health care programs that utilize this data and information to manage the total care for populations using predictive models to improve the deployment of resources and initiatives to improve the cost of care and health of populations. The presenters will also look at how providers of care are approaching mental health co-morbidities to manage physical health and social determinants of health in the management of patients.
At the conclusion of the session, attendees will be able to:
- Define non-physical health factors impacting the total cost of care
- Identify sources of information and how it can be applied to provide better healthcare outcomes for people
- Explain how information beyond claims data might be utilized to improve predictive models
Responsibilities & Duties
Moderator - introduce speakers, manage session timing, moderate audience questions
Presenter - develop material and deliver presentation
Time Commitment
- 5-10 hours for presenters
- Moderator time commitment will be minimal.
- Depending on the subject matter and the time needed to develop the materials for the session, the time commitment may be more or less than listed above.
